Entry form & Liability Waiver

Download the Trial and Seminar application form. Also all trial participants must sign and submit either one or both of the following two liabilitywaivers, ADULT Waiver and/or PARENTAL Waiver as applicable.

These forms together with applicable fees are to be submitted to the Trial Secretary, Adam Mercer (see above), by or before August 25th.

Please note the liability waivers must be initialled, signed and witnessed, as noted in the forms and provided as original paper copies.
